What is SurvivorLink™?

SurvivorLink™ is a website designed to help survivors of childhood cancer communicate with their doctors and nurses. If you, or your child, are a survivor of childhood cancer SurvivorLink™ can help you share information about your cancer treatment and risk for future health problems after cancer treatment. SurvivorLink™ is a safe and secure place for you to store your health records. You can allow your doctors and nurses to view your health records on the website. This will help your doctors, and any doctors you see in the future, understand your cancer diagnosis and treatment.

Survivors of childhood cancer can develop health problems after finishing cancer treatment. These health problems are called late effects. Survivors are at risk for certain late effects based on the type of treatment they received. You can find out about late effects that you might be at risk for by visiting a cancer survivor clinic. All childhood cancer survivors should visit a survivor clinic. During a survivor clinic visit you will:

  • Learn about possible late effects of your cancer treatment
  • Have tests done to screen for those late effects
  • Get a Survivor Healthcare Plan document

Survivor Healthcare Plan

A Survivor Healthcare Plan (SHP) is a document with information about your:

  • Cancer diagnosis and treatment
  • Late effects (health problems) that can occur after your cancer treatment
  • Tests to screen for these late effects (health problems)

The SHP will tell you which health problems could occur after your cancer treatment. It will also tell you the tests you will need to screen for these problems and how often you will need to have certain tests. It is also helpful for all of your doctors (pediatrician, family practice, cardiologist etc) to have access to this information. Your doctors and nurses can use the SHP to learn about your cancer, cancer treatment, and late effects you are at risk for.

Storing Your SHP on SurvivorLink™

SurvivorLink™ is a place for you to store your SHP and any other health documents. By saving your SHP on SurvivorLink™ you won’t have to worry about losing the copy that you get from your survivor clinic. Your doctors can view your most recent SHP online so you won’t have to bring them a new copy. If you see a new doctor, you can allow them to view your SHP on SurvivorLink™ so they can quickly learn about your medical history.

On the 'Patients and Their Families' section of SurvivorLink™ you will find:

  • How to find a survivor clinic
  • Insurance information
  • Teaching sheets about late effects for childhood cancer survivors and their families

Your doctor or nurse can use the Provider section of SurvivorLink™ to:

  • Learn more about late effects of cancer treatment
  • Earn continuing education credits by reading modules or watching a video
  • Access links to the website with national guidelines for screening tests after childhood cancer

SurvivorLink™ has been designed to:

  1. Increase awareness and knowledge about the life-long healthcare needs of pediatric cancer survivors
  2. Increase communication about the specific healthcare needs of individual survivors between:
    1. Survivor and Family
    2. Cancer Survivor Team
    3. Primary Care Provider
    4. Specialty Doctor

SurvivorLink™ has been built by engineers, researchers, doctors and nurses at Children’s Healthcare of Atlanta, Georgia Institute of Technology and HIMformatics, LLC. Funding for SurvivorLink™ is provided by the Agency for Healthcare Research & Quality, the Lance Armstrong Foundation and the Georgia Cancer Coalition.
